CHNA participation is open to anyone interested in working with others to improve the health and well-being of local residents and communities.

CHNA's play an important role in addressing statewide public health priorities, including:

  • Eliminating racial and ethnic health disparities and their social determinants

  • Promoting wellness in the home, workplace, school, and community

  • Preventing and managing chronic disease

CHNA partners have the opportunity to:
