Our vision is optimal health and well-being for all. We work to achieve that through grantmaking, collaborating, educating and supporting the work of member organizations throughout our service area.

Based on a 2011 Needs Assessment conducted within our 13 communities, CHNA 20 currently focuses on the areas of Substance Abuse, Mental Health, Chronic Disease and Wellness, and Access to Care as priorities for improved public health. However, our work and priorities are continually responsive to the changing needs of those we serve.

We believe that the path to sustainable change for the health of our communities lies in multi-sector, collaborative, interdisciplinary efforts that address the roots of health disparities, not the symptoms.
